Certificate of TB(tuberculosis) test for long-term visa

All applicants of long-term visa(90 days above) from the mentioned 35 countries (including Azerbaijan) below will be required to submit Certificate of TB(tuberculosis) Screening issued from hospitals designated by the Embassy.

※ Submission of Certificate of TB(tuberculosis) Screening will be waived for applicants of Diplomat(A-1), Official(A-2), and Agreement(A-3) visa types. Certiciate of TB(tuberclosis) Screening will be waived as well for children under 6 years old, pregnant women

[Attention]

1. Validity of the certificate is 3 months

2. Certificate of TB(tuberclosis) screening issued by designated hospitals are accepted only. 

3. Certificates of TB scrreening using the attached form will be accepted only. 

4. TB treatment history, Signs &Symptoms suggestive of TB and Chest X-ray result are mandatory(please see the attached file) 

5. Photo of applicants should be attached at the Certificate. The photo should bear the dry seal/stamp of the hospital. 

6. Embassy can ask the applicant to submit laboratory test result with the certificate of TB(tuberculosis) screening. 

7. Please show your international passport at the hospital. 

※If result of TB test is positive, the visa application is automatically denied.
