If you wish to apply for a Korean visa, please download one of the attached documents (all of which are identical application forms in different file formats) and complete it either on your computer or by hand. However, the applicant must personally sign the signature field on the last page by hand. Please ensure that all information is written in block letters.

• ●A One Standard Size Photograph is required, and printed photographs will not be accepted. Please ensure that the one standard size photograph is affixed to the application form. There is no photo booth at the consular section.

• ●Please make sure to complete the visa application form before visiting the consular section. If the form is not completed and/or a photo is not provided, your application will not be accepted on the day of your visit, in order to ensure the smooth processing of consular services for other visitors.

• ●The applicant must complete this form fully and accurately.

• ●The applicant must write in block letters, either in English or Korean.

• ●For multiple-choice questions, the applicant must check all options that apply.

• ●If the applicant selects "Other," please provide additional information in the space provided.

Si vous souhaitez demander un visa pour la Corée, veuillez télécharger l'un des documents joints (tous sont des formulaires de demande identiques mais dans différents formats de fichier) et le remplir soit sur votre ordinateur, soit à la main. Cependant, le demandeur doit signer personnellement à la main dans le champ prévu à cet effet sur la dernière page. Veuillez vous assurer que toutes les informations sont écrites en lettres majuscules.

• ●Une photographie au format standard est requise, et les photographies imprimées ne seront pas acceptées. Veuillez vous assurer que la photographie au format standard est fixée au formulaire de demande. Il n'y a pas de cabine photo au consulat.

• ●Veuillez vous assurer de remplir le formulaire de demande de visa avant de vous rendre à la section consulaire. Si le formulaire n'est pas rempli et/ou si une photo n'est pas fournie, votre demande ne sera pas acceptée le jour de votre visite, afin d'assurer le bon déroulement des services consulaires pour les autres visiteurs.

• ●Le demandeur doit remplir ce formulaire de manière complète et précise.

• ●Le demandeur doit écrire en lettres majuscules, en anglais ou en coréen.

• ●Pour les questions à choix multiples, le demandeur doit cocher toutes les options qui s'appliquent.

• ●Si le demandeur sélectionne « Autre », veuillez fournir des informations supplémentaires dans l'espace prévu à cet effet.

Als u een visum voor Korea wilt aanvragen, download dan een van de bijgevoegde documenten (allemaal identieke aanvraagformulieren in verschillende bestandsformaten) en vul het in op uw computer of met de hand. De aanvrager moet echter persoonlijk met de hand ondertekenen in het handtekeningenveld op de laatste pagina. Zorg ervoor dat alle informatie in hoofdletters wordt geschreven.

• ●Een standaardformaat foto is vereist, en afgedrukte foto's worden niet geaccepteerd. Zorg ervoor dat de standaardformaat foto aan het aanvraagformulier is bevestigd. Er is geen fotohokje op het consulaat.

• ●Zorg ervoor dat u het visumaanvraagformulier invult voordat u de consulaire afdeling bezoekt. Als het formulier niet is ingevuld en/of er geen foto wordt verstrekt, wordt uw aanvraag op de dag van uw bezoek niet geaccepteerd, om een vlotte afhandeling van consulaire diensten voor andere bezoekers te waarborgen.

• ●De aanvrager moet dit formulier volledig en nauwkeurig invullen.

• ●De aanvrager moet in blokletters schrijven, in het Engels of Koreaans.

• ●Voor meerkeuzevragen moet de aanvrager alle van toepassing zijnde opties aanvinken.

• ●Als de aanvrager "Overig" selecteert, geef dan extra informatie in het daarvoor bestemde veld.
