1.7. National Identity Number

This refers to your national identification number, which is different from your passport number.

For Lithuanian nationals, it is written inside the Lithuanian passport.

If you are not Lithuanian, you may find this number on your passport or national ID card issued by your government.

6. Education

6.1. Please mark the highest degree or level of education you have completed.

   *  If you are currently studying for a Bachelor’s degree, it means you have not yet completed it. Therefore, your highest completed level of education is High School/Gymnasium.

   * If you are currently studying for a Master’s degree, your highest completed degree is Bachelor’s Degree.

Note: You will provide details about your current education status in Section 7 (Employment).

7. Employment

7.1. For your current circumstances, please read the options carefully. You will see "Student" listed — select this if it applies to you.

   * "Employed" if you are employed by model agency (= mother agency) in Lithuania.

7.2. Provide full details of your school, college, or university, including the name and address.

   * details of your mother agency if you marked "Employed" as model.

8.2. If your intended Period of Stay in Korea is longer than 90 days, please indicate the duration in months, not days.    

   * For example, if you plan to stay for 121 days, write 4M or 5M.

8.3. Please remember that due to the time difference between Lithuania and Korea, your arrival date is usually one day after your departure date from Vilnius. Check your flight schedule to ensure accuracy.

   * Please wrtie approximate date even if you haven't bought the tickets.

9. Details of Inviting Party

You are being invited by the host university/model agency in Korea. Please fill in their information in this section.

The required details can be found in the university’s/agency's Certificate of Business Registration (사업자등록증명) — please refer to this document when completing the form.
