F-4 Korean Heritage Visa
* Please apply for visa no more than 90 days before your scheduled departure*
* We do not make any printouts or photo copies.*
Completed visa application form with one coloured, passport-sized photo (recent & white background)
Fill in your name as shown on your passport.
Paste your photo on the space provided under Section 1.
Application will be REJECTED if there is incorrect/missing information.
Passport with at least 6 months validity and one blank "visa" page (original & copy)
Passport must be kept at the Embassy during the processing of your application.
Please arrange your schedule accordingly.
Additional documents based on purpose of visit
Dutch citizenship certificate of the applicant(original & copy)
Recent copy of the Family Relationship certificate(가족관계증명서-Gajokgwangyejeungmyungsuh) and the Basic certificate(기본증명서-Gibonjeungmyungsuh) or Family Register certificate(proving the applicant’s citizenship has been renounced)
A scanned copy(printed) is acceptable.
You can also apply the documents at Consular Section.(It takes about 3-4days to get issued.)
(This applies only if applicant is under 38 years of age) Applicant’s parents’ passports (photocopied) and Dutch citizenship certificate(they also should renounce ROK citizenship) or Permanent Residency card which is valid
A scanned copy(printed) is acceptable
The proof that you have reported the 'Loss of Korean citizenship' in the past(국적상실신고접수증)
If you didn't apply for report the loss of Korean citizenship, you can also apply F-4 visa and Korean citizenship renouncement (국적상실)at the same time. For 국적상실, please click here and refer to the website.

※ Important information for F-4 Visa applications and other people of Korean heritage:
Among dual citizenship holders who were born outside Korea but whose names were included on their family register(Hojokdeungbon), only those who have renounced their Korean citizenship can apply for F-4 visas. Men aged 18 to 37 who did not renounce Korean citizenship before they reached 18, cannot renounce their Korean citizenship unless:
1. Have an exemption from military service, or
2. Complete their military service
Please note that men aged between 24 to 37 who did not report their loss of Korean citizenship
before the age of 18, should apply at the Embassy for deferring military enlistment.
