Requirements For Seasonal Workers (C-4, E-8) with Visa Issuance Numbers

1. Application Form  ※ Please use the attached form.

2. Original Passport, Photocopy of Passport Bio-page (page 2), 1 piece of passport size colored picture (Please attach on the application form)

3. Medical Certificate (issued within 6 months from the date of visa application)

  ※ No designated hospital.

  ※ General health checkups should include lab test results of AIDS, Syphilis, and Drug.

4. Certificate of TB (tuberculosis) Screening (issued from hospitals designated by the Korean embassy within 3months)

  ※ Only those examined at the designated hospitals are valid.

  ※ Refer to attachment for the list of designated hospitals and Certificate of TB (tuberculosis) Screening.

5. NBI Clearance Certificate (E-8-1~4 visa applicants only)

  ※ Issued within 3 months from the date of visa application.

■ Visa Fee:

- C-4 Visa: PHP 2,000

- E-8 visa: PHP 3,000

■ Processing period : 5 working days
