Quarantine Exemption Certificate for the Fully Vaccinated to visit their immediate family living in Korea (updated on Oct 1. 2021)
Starting July 1st, 2021 those who have completed COVID-19 vaccination overseas are eligible to apply for a quarantine exemption certificate to visit their immediate family living in Korea.
[Eligibility]
1. Do you have immediate Family who are permanent resident in Korea or have a long-term visa?
- The immediate family means your grandparents(your father’s parents or your mother’s parents), your sons and daughters,
grand sons and grand daughters, parents in law, your sons in law or daughter in law.
- Your brothers, sisters, uncles and aunts are not included in the above category.
2. Are you fully vaccinated in the US?
- The fully vaccinated means;
- people get the vaccines approved by the Who
- AND 15 days has passed since you receive the final shot.
※ Cross innoculation is not allowed.
- Children under the age of six are eligible to apply for the Exemption Certificate without the COVID-19 vaccination.
3. Is your flight going to be departed from one of the regions that the Korean Consulate in Anchorage?

1. Quarantine Exemption Application (attached 1)
* Please check “Fully Vaccinated Overseas” on the top left.
* Please check “agree” in the middle and do not forget to put your name and signature.
* The "status of stay" is for VISA type that you have.
※ If you don't have KOREAN VISA and just visit your family within 90 days, you can write down "B-2" which means VISA Waiver program.
2. Agreement to the Terms and Conditions (attached 2)
* Please check “agree” on the bottom and do not forget to put your name and signature.
3. Pledge of Authentlclty(attached 3)
* Please check “agree” on the bottom and do not forget to put your name and signature
4. Copy of valid passport of the applicant
5. Government-issued document to prove family relationship with the immediate family member in Korea
* If your immediate family have Korean nationality;
- (e.g.) Korean family relationship certificate (가족관계증명서), Korean family registry document제적등본), US. birth certificate, US. marriage certificate, etc.
* Korean official documents should be issued within 90 days of your quarantine exemption application date.
* If your immediate family don’t have Korean nationality;
- the Korean Alien Registration(외국인 등록증), or Korean Residence Card
- copy of their visa
6. Copy of your Vaccination Card of the applicant
7. Copy of your Vaccination Card of the applicant
8. Proof of Alaska Residence (Driver's License, etc.)

[FAQ]
1. How long does it take to issue the Exemption Certificate?
- About 3 to 5 days (based on working days)
2. How long is the Exemption Certificate valid?
- 30 days after the issue.
3. Should I resubmit the application if my flight schedule is changed?
- The exemption certificate is valid within 30 days from its date of issuance. So you can depart from the US anytime within 30 days after the issue without re-submitting the application.
