   - Visa Application Form(attached)
   - 1 photo (2” x 2”) to be attached to the application form

     ※ Photo must be taken within the last six months
   - Passport: original & copy(personal information page)

     ※ more than 6 months remaining on the expiration date
   - Non-U.S. citizen: U.S. visa or green card (original & copy)
   - Additional Documents Required by Visa Type(attached)
     ※ After reviewing your visa application, additional documents may be required beyond the list of additional documents.
