Application for Family relationship certificate(가족관계증명서), Basic certificate(기본증명서), Marriage certificate(혼인관계증명서), Family census registry(제적등본), etc. 



o How to apply?

 ① By mail

      (Once you have all the documents ready, you can mail them to our office at 3500 Clay St., San Francisco, CA 94118)

o Information that you need for the application

 ① Korean resident number (주민등록번호) or

 ② Korean family registry address (본적)

 (In case you are applying for family census registry (제적등본), you must know your Korean family registry address (본적) and the head of your Korean family registry address (호주).)





o Required documents

 ① Application form (가족관계 등록사항별 증명서 교부 등 신청서 *Form attached)

      - Please fill out all the sections marked with 

      - Your name on the application form should be your Korean name in Korean.

      - You must provide a U.S. phone number in the application form in case we need to contact you for further information.

     - If there is any missing section/problem in the application and if you do not provide your phone number, then we will return your application without processing it.


 ② 1 copy of your valid ID such as passport, driver's license


 ③ Proof of name change (If applicable)

      - 1 copy of the petition for name change (In case your name has changed due to naturalization)

      - 1 copy of the marriage certificate in case your last name changed due to marriage

      - If your name changed due to adoption, you should submit the legal proof of adoption and name change.  



 ④ Processing fee ($1.00 per copy, cash or personal check only (Pay to : Consulate General of the Republic of Korea in SF))


 ⑤ (If you want to receive your certificates by mail) Self-addressed return envelope with a stamp or a prepaid-label/postage (If you want to put forever stamps, please put at least 2 stamps.)


      - If you prefer to pick up the certificates, you do not have to provide a return envelope. We will call you to the phone number that you provide on the application form for pick-up.


 ※ You can apply for the certificates of your parents, your sons and daughters, and your spouse as long as you know their Korean resident no. and/or Korean family registry address. Also, your relationship with them must be already registered in Korea. If not, you should submit a document to prove the relationship such as U.S. birth certificate, marriage certificate, etc.



o Processing time

 - 3~4 business days (not including the shipping/mailing time)

 - Once the certificates are issued, we will call you to the phone number that you provide on the application form for pick-up. Or we will send out the certificates with the return envelope that you provide.

 ※ If you do not pick up your certificates within 6 months of their issuance date, we will shred them without notice due to the private information protection law.
