Notarization Issuance Guide

Application for Notarization on Academic Records

• ●Requirements:

• ○Academic records

• ■The main Documents must first be certified by the Ministry of Foreign Affairs in Nigeria.

• ■Affidavit from High Court – with covering letter

• ○Valid International passport and COPY

• ○Fee $4(6,000 – Bank transfer)

* Bank account details will provided once your notarization application has been reviewd and approved.

• ○Additional Requirements for authorized representative : Letter of Attorney, Valid ID Card

Application for Notarization on Sworn Statement/Affidavit

• ●Requirements:

• ○Sworn Statement or Affidavit

• ■The main Documents must first be certified by the Ministry of Foreign Affairs in Nigeria.

• ■Affidavit from High Court – with covering letter

• ○Valid International passport and COPY

• ○Fee $4(6,000 Naira – Bank transfer)

Application for Notarization on Police Character Certificate

• ●Requirements:

• ○Police Character Certificate

• ■The main Documents must first be certified by the Ministry of Foreign Affairs in Nigeria.

• ■Affidavit from High Court – with covering letter

• ○Valid International passport and COPY

• ○Fee $4(6,000 Naira – Bank transfer)

Application for Background Check (Criminal Records) Certificate

• ●Requirements:

• ○Original Passports (passport used in Korea, new and valid passport) and COPY

• ○1 piece Passport-sized colored picture (3cm x 4cm) taken within the last 6 months

• ○Application Form

• ○Alien Card or Certificate of Alien Registration

• ○Fee $0

• ●Nigerians and other foreign applicants must fill-up the English part only of the Application Forms.

• ●Personal appearance of the applicant is required when applying for police clearance in the Office.

• ●Please submit the copy of the old passport with the arrival and departure stamp from Korea.
