Please note that not following the proper procedure to apply for a visa can automatically result in denial of a visa. Furthermore, any verbal abuse or unlawful behavior and actions are grounds for denial of visa issuance and may prevent future visa applications.
THIS VISA IS FOR
○ A person working for USFK or non-military SoFA status personnel under the SoFA (Standard of Forces Agreement).
○ Civilian Component, Invited Contractors by the U.S. Department of Defense.
○ Immediate family members of the above
○ This visa will be issued with a single or multiple entry and the duration of stay is until the end of the official assignment.
VISA PROCESS
○ If the applicant and the dependent family members will be moved to the U.S. Military Base in Korea directly via U.S. military aircraft, can apply for this visa after arriving in Korea without this visa process. In this case, please discuss the process with the Officer or Commander of USFK.

: Color (on photo paper, natural color, 2x2 inches, taken within 6 months)
3. Original Passport and a copy
: Official passport is required (A general passport is acceptable for special cases or family members only.)
: (If applicants apply for the visa by mail) Please send the notarized passport copy instead of the originals.

4. Visa Fee (Cash / Credit Card / Money Order)
: No charge (Waived)
5. Official Work/Travel/Dispatch Order
: P.C.S (Permanent Change of Station) Order / T.D.Y (Temporary duty) Order / LEAVE Order / ETC.
* 'Official Stations' or 'LEAVE address' or 'Assigned To' MUST be in KOREA.
* Reporting Date and the Arrival Date should be Valid.
* If the order has only APO NO, It MUST start the number of five digits APO NO with 96.
6. U.S. Military I.D
* USFK-PASS ID, Army Civilan Affiliate is NOT for valid I.D. for this visa.
7. Family Relation Certificates (If accompany)
: (Non-U.S. citizen) Orignal Valid U.S.Long-term Visa or U.S. Permanent Resident Card(Green card) and a copy
: (For Spouse) Marriage Certificate
: (For Child) Birth Certificate
* An individual original passport and application is required for each applicant.
* The name of each accompanying family member has to be on the Order.
* Don't forget to sign before you submit the application
ADDITIONAL INFORMATION
○ Korean ancestry applicants may be required additional nationality documents(기본증명서, 제적등본, 가족관계증명서) to verify dual citizenship. (Proof of loss of nationality, renunciation of nationality, etc.)
: If the applicant has dual citizenship, is NOT eligible to apply for a visa.
*The Applicant may issue Korean Passport instead.
○ If any information or documents submitted along with the visa application are verified as false statements or falsified, will cause a denial of visa issuance and may prevent future visa applications.
○ Sticker-type visas are no longer issued. The visa grant notice is the actual visa that must be brought for entry to Korea.
