• ●Basic Certificate (should be reported Korean nationality loss)

• ●Original FBI IDHSC (Identity History Summary Checks) and Federal Apostille confirmation by U.S. Department of State Office of Authentications
  * Must be issued within 6 months before the date of visa application
  * Must receive FEDERAL Apostille; state Apostille not accepted.

• ○All non-US citizen applicants must provide the Criminal History Record (notarized and/or Apostilled) issued from their home country in addition to the FBI check.
